Insurance Info

All major insurances accepted except for the following health insurance plans:

• Humana MedAdvantage HMO (We do take Humana Choice PPO)
• Kaiser Permanente HMO
• Veteran’s Affairs

If the patient has another HMO (such as United HealthCare/WAHLOP /WA Apple Healthcare), we cannot see the patient unless the patient or family contacts the insurer and assigns us as the named Primary Care Provider prior to being enrolled as our patient.
